Exim can call SpamAssassin before accpetance.

And since you're using Debian Squeeze, you should look again at the
mailscanner package: 'apt-cache show mailscanner' shows that the package
has been qa::orphaned. :-/ And indeed the package has been dropped from
Sid and Wheezy, so if you want to continue using it you should probably
get the last-known package from snapshots.debian.org and roll your own
version of it.
