Looks pretty good, but I found something switched. Pick any chapter
and click on it. Now look at the link for the chapter heading (it
actually points to the table of contents), and look at the link on the
page bottom to the table of contents (it actually points to the
chapter).

I looked at the diff and it wasn't immediately obvious to me how to
reverse that, so I can only point at the problem and not be very
useful for suggesting a solution. :-/
