I don't understand this patch at all. According to my man pages, fileno is
defined in stdio.h, and indeed it is on my Linux box. Both stdio.h and unistd.h
are #included by pcregrep.c. Also S_IFMT etc should be available just by
including sys/types.h and sys/stat.h as documented for the stat() function.

Is this perhaps something odd related to c99? I will see if I can test that
sometime, but it seems strange that it would affect #included files.
