I wouldn't exclude the notion that it's an exploited box trying to
hack someone's account. Look to see if the username changes with some
regularity (brute forcing usernames). If it's the same mailbox over
and over again then it likely is not an exploited box, but it is
running unfriendly software like you mentioned.
