This piece of code

set acl_c_ratio = ${eval:$acl_c_bad_rcpt_count_60 * 100 /
$acl_c_out_sender_rate_60}

Fails when the variable $acl_c_out_sender_rate_60 = 0

as it should - but ....

It fails to show and kind of error in the logs. Should log a divide by zero
error.
