This doesn't currently consider the >128 scenario where a shell munges a signal
exit value.

I think this is okay until proven otherwise:

* if the shell is from the invocation of the command by Exim, we discourage
that anyway and this is another discouragement
* if the shell is an intermediate script then it can assume responsibility
for returning EX_TEMPFAIL
