Ted: it's for the outbound case, and the patch should address it, but I don't
have a PIX to test against and am not comfortable committing this until someone
verifies it actually helps.

The non-TLS writing happens with write() calls and Exim doesn't use
TCP_CORK/TCP_NOPUSH; Exim disables Nagle (TCP_NODELAY). AFAICT, both the TLS
implementations avoid buffering past the return of the write call.
