[exim-dev] ChangeLog numbering

I'm updating doc-txt/ChangeLog as part of the documentation update for
the dynamically loadable modules.

There seem to be two numbering schemes in use:

(a) everyone numbers their changes from 01, per-committer numbering
(b) shared numbering, the initials are just a tag

Which *should* be used going forward, please?
