Need to look at that - currently this is done by generating a batch of
extra docbook into the document which just drops xref elements for the
links - the docbook->html generation then converts those, so would need
to see how easy that is to modify (and it would probably happen across
the whole doc set not just the indexes).
I'm not well up on the xslt magic that Mike implemented to transform
the doc set so it may have to wait.
