My guess would be that Exchange fucked up and sent an unescaped
CRLF.CRLF sequence. This is consistent with the log messages you're
seeing, and if it's caused by a CRLF.CRLF sequence in the message that's
being bounced it probably explains why it doesn't happen all that often.

If you have access to the message that was being bounced and a copy of
the bounce message this could probably be easily verified.
