You need to set these with -oMi as well when using -bh. See Section 11.9
of the manual under $received_ip_address. 'When testing with -bh, the port
value is -1 unless it has been set using the -oMi command line option. '
exim -bh 127.0.0.2 -oMi 127.0.0.1.587
This frequently catches me out as well!
