The form only seems to want your mail exchangers IPs, together with a whole
load of information about what types of email you are sending.
Also on that page there is a link Yahoo!'s best practices page, half way
down that we find "What are some best practices when sending to Yahoo!
Mail?", I would suggest following the advice related to segregating email
sources by email type if at all possible.
