Patch to add ldap_cacertfile configure options

We had a need to allow exim to authenticate against our LDAP directory using
TLS and a self-signed certificate on the LDAP server. It can't currently do
this, so I've written the attached patch to implement the addition of a CA
certificate.

It works in our environment with ldaps:// URLs.

This implements at least part of bug 230.
