I would still strongly suspect that there is something such as a firewall
(possibly in software on the target box) that is breaking this - as far
as I know MS have not as yet been accused of breaking pipelining on
Exchange where they advertise it.

In the meantime, on your exim box modify the SMTP transport and add
hosts_avoid_pipelining = my.exchange.box

using the appropriate name of the target box. This can be a list,
or even a wildcard - see docs:-
