One of two things:

 (1) You're hitting bug 401, caused by bug 402, fixed in Exim 4.64;
     see ChangeLog for details.  We're now at Exim 4.69.


(2) Your transport_filter script is returning non-zero.

Debugging, perhaps with -d+transport, will show more information.
