I just did this using a system filter (named myfilter here) and containing:

# Exim filter
if $h_to: contains "xxxxxx" then
headers remove "From"
headers remove "Reply_To:"
headers add "From: yyyyyy qqqqqq <zzzz@???>"
endif

Add to the main config file something like:
system_filter = /etc/exim/myfilter
system_filter_user = exim
