Exim has support for several anti-virus products. The open source
ClamAV product:

is very effective and can be used with additional signatures. The
phishing and spam signatures from Sanesecurity:
http://www.sanesecurity.com/clamav/
are extremely useful.
We have a site licence for Sophos, a commercial product. Use sophos
via the sophie daemon:
http://www.clanfield.info/sophie/
We check all email with both Sophos and ClamAV.
