If you are hosting websites on the server, look for insecure phpmail
enabled websites or insecure formmail.pl. Just because SPAM is going
through the server it may not be Exim that is being exploited.
Finding insecure formmail.pl (or derivatives) and php mail() are
painful having experience similar situation.
