[exim] Likely cause of duplicate emails

Somewhat randomly and occasionally I'll get a complaint about duplicate
emails. I think it is likely related to once side sucessfully sending
the email but thinking it failed and retrying.

I wish I had more information than this. I was just wondering if anyone
else had noticed this and can say "here's how you fix it".
