I have made notes to improve the documentation for the next edition. The answer
is that smtp_accept_max_per_host is entirely independent of
smtp_accept_reserve. The order of checking is: smtp_accept_max,
smap_accept_max_per_host, smtp_accept_reserve. In other words, it doesn't look
at smtp_accept_reserve until it has checked that the absolute limit and the
per-host limit are not exceeded.
