P.S. In my experience the exchange smarthost settings on the virtual
smtp server work more reliably as "IP literal" entries such as
[192.168.1.1] instead of hostnames such as "exim.example.com".

Also see the corresponding exim settings for "allow_domain_literals"
settings and "rfc1413_query_timeout" in the default configuration file
(which is also available here):

Also, also look out for underscores in the exchange servers hostname as
these are not generally allowed in the initial HELO string:
