In some circunstances, exim rejects the message without retrying to send it or
put it in the queue:

As you can see, the email is rejected after a timeout without even retrying or
put it in the queue for later processing...
sent at 11:12 rejected at 11:15 same day.
happened with exim 4.60 and 4.65
