Function to do the dirty work

I haven't figured out whether the lenptr and sizeptr arguments are actually
needed (I looked at rfc2047.c when I wrote this).

In contrast to rfc2047_decode2(), my function gives up if iconv fails, because
it's not very probable that can produce anything useful anyway.

After a closer inspection, I don't think it's reasonable to fit this in
expand.c cleanly as an expansion operator; it will have to be an expansion
item.
