As usual, this is caused by broken DNS setup at the domain in question.
In this case the problem is that bell-mortgage.net's NS records point to
ns1.kellywhitedesign.com and ns2.kellywhitedesign.com, which are also
listed as the nameservers for kellywhitedesign.com, but both of them
reply with NXDOMAIN for queries for ns1.kellywhitedesign.com and ns2.
The effect of this is that an initial query or two succeeds thanks to
the glue records returned by the .net / .com servers, but as soon as
your local caching resolver looks up the ns[12].kellywhitedesign.com
records, it gets NXDOMAIN which it will cache, and while it has that
cached it will return SERVFAIL for any queries for domains served by
those nameservers.
