The Debian Exim setup is way different to "vanilla" Exim, which is what
we use on this list (mainly). The Debian Exim package is down the hall :)

Especially the README.Debian, which states that you should be using
exim4-daemon-heavy for AV/AS integration. And from there you can follow
through to the Debian Exim mailing list archives (searchable with
Google) which contain many a suggestion.
All that said: did you restart the clam daemon after fiddling with the
groups? Did you tinker with Exim or ClamAV's log level to see what's
happening?
