The only thing that seems to relate:

shows that phrase related to a 550 REPLY.
In RFC 1893 it is a general-purpose catch-all for something not more
specifically determined.
Can you replicate the occurrence?
Are there entries for that/subsequent messages already in your logs that show
what led up to it?
If not, enhance your logging.
