You need to create a procmail router and a corresponding transport.
Ask Google for examples. I'm sure you'll find lots on the Web.

procmail is used for incoming mails only, so the second "interaction"
is not necessary.

If you want to be able to send mails "locally" (from local user A to
local user B) you need to integrate procmail.

You didn't get it: exim uses another place to store the locally
deliverd emails than your procmail configuration does.
Nevertheless, it's useful to tell exim about which domains to consider
as local_domains.
