It's not compiled into the default package. You need to install
exim4-daemon-heavy to get exiscan compiled in, as well as many other
useful options.
If you have further debian-specific questions, please read
/usr/share/doc/exim4-base/README.debian.gz, and take them to the Debian
exim user list.
