Oh, sorry :(
adding AllowSupplementaryGroups to the /etc/clamd.conf file fixed it.
Not sure why it was getting access denied if the user clamav was added to
the mail group (which owns the directories and files), but adding the
AllowSupplementaryGroups, fix it.
Thanks again for your time and help.
PST: I don't see in any logs enties about clamd, but I checked from
www.webmail.us sending eicar virus tests, and they don't arrive to the
mailbox. So I think it's working fine, just that I don't see anything about
clamd in logs /var/log/exim/* or /var/log/maillog
