Do you have a specific need to use sa-exim, or just a general wish to
perform content scanning at SMTP time? Assuming the latter, you don't
need to do much at all - recent versions of Exim include an
alternative to exim-sa (formerly called 'Exiscan') which interfaces
with SpamAssassin for spam scanning, and with various antivirus
scanners for malware scanning. And several other useful things.
Get a vanilla Exim and check out the docs, especially chapter 40. Then
check out the wiki for more examples.
You can probably make exim-sa work for you as well, but I don't
believe it's well supported now that Exiscan is built in.
