Is this something I need to set, or is this something they've changed on
their mail server ?
Only ask, because we've made no changes to our server, and given its only
one client having the problem, I'm reluctant to make a change on our config
settings just for one person.

Smells like the notorious rfc1413_query_timeout (aka ident aka auth),
which is also 30s. You may change this to something lower (I have 3s) or
even 0s if you're not interested in ident lookups at all.
Anyway, it's not Exim's fault that the remote end blocks ident requests
(proper behaviour is "reject", not "blackhole", if they don't want to
run a identd). And 30s connect timeout is much too low.
