This could perhaps be done by extending the local_scan() idea and providing a
"standard" module which interfaced to milter.

I have done nothing about it, and I suspect nobody else has either. As
for the difficulty, as I know nothing about milter, I can't even make a
guess (hence the "?" in the WishList item).

The local_scan() idea is probably not right nowadays, given that we now
have other kinds of content scanning at ACL time.
