Should work, I think. Are you sure the clients are connecting from the IP
that natsp-43-netra20.ice.co.cr resolves on your machine?

Would be helpful to run a test session with 'exim -d -bh 200.91.77.43' (or
whatever natsp-43-netra20.ice.co.cr resolves to on your machine).
