Exim 3 is not supported. You probably need to check your hostname lookup
options. Exim 4 does the reverse DNS lookup for $sender_host_name
automatically.

You should not change received_header_text without reading and
understanding the syntax in section 4.4 of RFC 2821.

There isn't any point in obfuscating your private network addresses.
