OK. First, sorry, but my English is not enough good. LMTP is used only in
"hop 1", as you said. The process is:

Internet -0-> Exim_1 -1-> Kaspersky -2-> Exim_2 -3-> delivered
         SMTP      LMTP           SMTP      disk
                     |                        |
                     5                        4 (NFS error -timeout-)
                     |                        |
                   queue (Exim_1, ID_1)     queue (Exim_2, ID_2)


Finally, when queue runner starts, final user receives the mail twice,
because it was queued by both Exim processes.
