How about (for 4 IPs):
    interface = ${lookup {${nhash_4:$message_id}} \
             lsearch {/usr/local/exim/my_ips} {$value} {1.2.3.4}}

where my_ips contains:

0: 1.2.3.20
1: 1.2.3.22
2: 1.2.3.34
3: 1.2.3.99
