I had a simular problem when my permissions in the spool area became
corrupted (new sysadmin was "trying" to make the system more secure and
broke it). I tracked it down to exim not being able to correctly write to
the directories to note completed deliveries.

You are quite sure its not still on the queue?
That exigrep error means that it can't find the message completed tag in the
log file, so normally its either still on the queue or the entry is in a
different log file.
