Header variables correspond to fields from the message header. Other
variables do not, e.g. they relate to the message envelope or the user
receiving the message etc. Many of them are listed in section 3.3 of the
Exim filter specification.

Yes, unless the header is added later by the message store (e.g. the IMAP
server).
