The answer is that no one has wanted the feature enough to write it. Given
that has been the case since IMAP became "real", it would seem that the
demand is low.

Marc, it would seem you have about three choices...write it yourself, induce
(with or without monetary help) someone to write it for you (I am not a
candidate), or make it a formal feature request and wait until it makes it
to the top of the Exim to do list. [The good news is you have a working
solution while you wait.]

Since it appears there can be more than one IMAP server involved, design of
the authenticator configuration for Exim could be tricky. That should be in
there, even if in your case your IMAP server talks to others per its
configuration to get "remote" authentications, and everyone is known to your
server by that means.

I don't recall any clients which speak only CRAM-MD5...as has been pointed
out, CRAM-MD5 and other mechanisms which work similarly (or will work
similarly when invented) cannot be used. There are clients which prefer
CRAM-MD5 if offered, but you wouldn't advertise it.
