If I am not mistaken, Exim knows only about 1 Maildir/ - the one in your
equivalent of local_delivery transport.
And yes, Exim can 'count' and use the whole size of that Maildir for
computing quota. It's a feature already in Exim.
You've referred to /var/mail/user and /var/mail/users - those are two
different directories, yes? So your spam deliveries are being handled
by another transport, yes? Exim will not honor the later. Perhaps you
should change the setup to have all mail and spam delivered to
/var/mail/user/
