Envelope. In theory, an MTA is concerned only with envelopes, just like
regular snail mail post handlers. (In practice these days, MTAs often
have to look inside to do various checks.)

There is no relation. The creator of the message can fill in whatever
they want to, though a Message Submission Agent (MSA) may enforce
certain constraints.
