First, don't start a new thread by replying to an existing message. It
screws up threading for all good mail clients.

It means that exim did not have permission to do something. It's
probably actually the fact that the user exim runs spamassassin as can't
read/write to the spool area.
Try running exim with debugging turned on for more info.
