You can probably acheive what you want by doing the following (untested):
headers_remove = \
.ifdef MY_SMTP_DISABLE_SENDER
Sender :\
.endif
.ifdef SA_EXIM_SMTP_HEADERS_REMOVE
X-SA-Exim-Blah :\
.endif
and remember to leave a blank line afterwards.
