If all messages arrive on the server unsigned, then you can arrange for
them to be delivered to a pipe transport which signs them and passes them
back to Exim for onward delivery. This is similar to some techniques for
spam scanning. See for example

If some messages arrive already signed, you can make the router
conditional on the content-type header.
