Check out Philip's perl script transport-filter.pl which use in the
transport_filter.

   remote_smtp_filter:
     driver = smtp
     transport_filter = /path/to/transport-filter.pl


So as Matt said use alterMIME and modify the Philip's perl script to suit
your needs.
