Re-read the debconf question. Relay_to_domains controls which domains
you accept mail for. Outgoing mail needs to be controlled by some
other means. _That_ one is actually the same as it was with sendmail.

I am pretty well aware of that mechanism.

After a successful update-exim4.conf, you shouldn't be an open relay
with that config.
