[exim] require smtp auth before relay, and use sasl2-bin

How is this done? I read everything I can find on google (including the readme
file of course in /usr/share/doc/exim4-base/README.SMTP-AUTH) and it's just to
vauge. I can't get the server to require authentication, and require that it be
done with TLS. How can I do it?

Everything else is finally working! This is the last step!
