I found the problem after some digging. The rejectlog said Exim could
not contact the pwcheck daemon.
When I upgraded cyrus-sasl from ports, it had removed (during make
deinstall) and recreated (make reinstall) the /var/pwcheck directory.
The directory ownership is cyrus:cyrus by default. As a result, Exim
did not have access to the socket, /var/pwcheck/pwcheck. I changed the
permissions back to mailnull:mail and was back in business.
