Aha! This appears to be the source of the problem.

The configuration had 'exim_path' set to the running version of Exim (which
probably had ALT_CONFIG_ROOT_ONLY set) and that was exec'ed instead of the
4.43 version. Correcting it in the test configuration file solved the
"Permission denied" message.
