Given where the guy lives, you'll find that his smarthost is likely to be
blocked all over the place.

http://revjim.net/comments/3734/ - which shows how to set up exim to send out
mail through an ssh tunneled smarthost (localhost:2525 tunneled to
localhost:25 on the smarthost shell / smtp server) ..

And what works for 2525 will work for 587 with very minor changes
